Registered Nurse - Care Team RN - RiverBend Medical Group- East Longmeadow- 40 hrs

Responsible for providing higher level clinical support to the health care team by providing superior nursing care services to patients in accordance with established R.N. job standards, policies, and procedures.
ESSENTIAL FUNCTIONS
Responsible for clinical job duties as designated by department
Acts as liaison/resource for patients to providers within the department
Acts as liaison between practices, departments, hospitals and home care
Provides support to the physician in managing complex patients. Assesses patients for acuity and anticipates treatment plan
Recognizes urgent/emergent situations. Take a lead role in initiating emergency response, treatment and stabilization of the patient.
Performs telephone triage as designated by department.
Work to achieve high levels of patient satisfaction by helping to resolve patient concerns or complaints.
Provides emotional support to the patient and family
Encourage active participation by all support staff in problem-solving strategies
Serves as a clinical resource to medical assisting, LPN staff and business staff..
Prepares patients for provider visits including but not limited to obtaining vital signs, a review of presenting complaints, allergy and medication reconciliation,
Perform back office lab testing and administer treatments per department protocol
Properly administers oral and/or injectable immunizations/medications within scope of practice and department protocol.
Insertion and maintenance of IV catheters
Assist providers with special exams and procedures
Accurately and appropriately, record patient information in Electronic Medical Record
Assist in the follow-up of patient results, per department protocol, at direction of a provider
Assist with coordination of patient care at office visits, on phone or through the electronic medical record
Coordinate follow-up appointments post hospital discharge/ER visits
Verbal communication and written documentation meets clinical and professional standards for level of practice
Encourage and enroll patients in the patient portal.
Provide outstanding customer service to internal and external customers.
Contributes to a positive work atmosphere.
Exhibits a spirit of teamwork in all interactions.
Maintain cleanliness of patient care areas, clinical equipment and medical instruments following designated protocols.
Travel between health centers may be required to attend meetings, training, or provide coverage.
Participates in departmental team meetings and training sessions.
Regular and reliable attendance in accordance with department needs and company policies
Adheres to all HIPAA privacy and security policies and practices. Reports violations and incidents they observe and cooperates in investigations as requested by management.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
Graduate of an accredited R.N. program. Current Massachusetts R.N. license required; CPR required; Advanced Cardiac Life Support certification preferred.
Three (3) to four (4) years of related clinical and/or hospital experience, or previous experience in a hospital or ambulatory care setting preferred.
Competent in a Windows-based computer system. Familiarity with Microsoft Office Suite.
Multi-tasking, problem solving and decision-making skills required. Ability to organize, analyze and interpret clinical data. High level of dependability and accuracy required.
Must have the ability to work well with others in a very busy, close environment as well as independently with minimal supervision. Puts others at ease by listening, allowing adequate time for interactions, and giving undivided attention. Shows receptivity to message being delivered. Gives attention to speaker; demonstrates non-verbal cues showing interest, and responds with information or questions, as appropriate.
Imparts information both verbally and in writing in a clear manner by sharing the appropriate amount and content of information so that it is understood by the intended audience. Chooses appropriate medium.
Responds to information and situations in a tactful and self-controlled manner. Maintains a calm composed manner during stressful times. Accurately assesses own strengths and weaknesses.

Flexibility to meet the coverage needs of the department.
WORKING CONDITIONS:
Lifting up to 25 lbs. required; bending and stretching required in storing of supplies and files distribution. Individual is expected to use proper body mechanics and request assistance as needed.
Job requires sitting and working at a computer for extended periods of time.
On rare occasions may be required to travel to other practice locations from time to time to provide coverage as deemed necessary by management.
REPORTING RELATIONSHIPS:
Reports directly to the Clinical Nurse Supervisor.
Has no responsibility for the direct supervision of any other employees.


Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
